Brent Lacy of MinistryPlace.net found it necessary to create a code of conduct for their youth group, outlining the expectations for how teens are to relate to each other and adult leaders. It also outlines their policy for how they address conduct issues when the code is broken.

It may not be necessary for every group to have a policy like this in place (mine doesn’t), but if you’re looking for guidance about how to be consistent when dealing with behavioral issues at youth group, maybe this can help point you in the right direction.
